Description & Requirements

Maximus is seeking a compassionate and detail-oriented SIS Assessment Coordinator  to support individuals with intellectual and developmental disabilities through the administration of the Supports Intensity Scale (SIS) assessment process. In this role, you will partner with individuals, families, providers, and community stakeholders to conduct comprehensive assessments that help determine the level of support services needed for successful community living.

This is a unique opportunity to combine your assessment, coordination, and relationship-building skills while making a direct impact on the lives of vulnerable populations. The ideal candidate is highly organized, comfortable working independently in both remote and community settings, and passionate about supporting individuals with diverse needs. You'll manage the assessment process from start to finish, including scheduling, facilitating assessment meetings, documenting findings, and maintaining ongoing communication with families and providers.

This is a field-based position serving a designated geographic region. Candidates must reside within Knox County or within a one-hour commuting radius to effectively support the assigned service area. Eligible business travel is reimbursed at the current IRS federal mileage rate, currently $0.76 per mile. Territories are assigned to help minimize travel while allowing you to provide essential services throughout your community. Work is completed during standard business hours (Monday - Friday, 8:00 AM - 5:00 PM), with availability required based on the individual's and families needs 3 % of the time on evenings and occasional weekends.

About Program: TennCare has contracted with Maximus to administer the Supports Intensity Scale for Adults (SIS-A) Assessments. This comprehensive planning tool, developed and maintained by the American Association for Intellectual and Developmental Disabilities (AAIDD), measures exceptional medical and behavioral needs, as well as the areas of home living, community living, lifelong learning, employment, health and safety, and social activities.

Essential Duties and Responsibilities:
- Responsible for completing medical and/or behavioral health assessments within contract requirements.
- Conducts on-site, video call and/or telephonic assessments to determine an individual’s needs for services and supports, eligibility, level of care or related outcome.
- Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures.
- Travel may be required based on program contract requirements.
- Performs other related duties as assigned.

- Preferred knowledge may include community support programs, long-term care assessment and level of care in medical, behavioral health or related programs. 
- Knowledge and understanding of medical and/or behavioral health diagnoses and prescribed medications.
- Ability to collect data, define problems, establish facts, and draw valid conclusions.

Minimum Requirements

- Education and licensure requirements are based on program contract requirements and are outlined in job posting.
- High School Degree or equivalent required.
- Minimum 2 years of clinical experience required.

- Must reside within Knox County or within a one-hour commuting radius of Knox County
- Bachelors degree in Psychology, Social work, Human Service, Nursing, Special Education, or Mental Health
- Minimum 2 years of experience working with individuals with Intellectual Disabilities or Developmental Disabilities (ID/DD) with Mental Health Conditions
- Demonstrated understanding and use of Person-First Language when interacting with individuals with disabilities, families, caregivers, and community partners
- Proficiency in computer skills, with the ability to navigate multiple systems and applications efficiently
- Proficiency in Microsoft Office applications
- Strong ability to multitask and manage competing priorities effectively
- Strong verbal and written communication skills
- Excellent organizational and follow-up skills
- Strong attention to detail and documentation accuracy
- Valid driver’s license and reliable transportation for field based work

Preferred Requirements
-  Minimum 2 years of experience working with individuals with Serious Mental Illness (SMI)
- Prior experience conducting Clinical Assessments
- Familiarity with medical terminology and diagnoses
